In September 2017, we announced that our Trust Gloucestershire Care Services NHS Trust and 2Gether NHS Foundation Trust (2gether) were planning to join. From October 2019, we hope to be one organisation providing integrated community mental health, physical health and learning disabilities services. The new Trust will be called Gloucestershire Health and Care NHS Foundation Trust.

We are already working closely together but the benefits we really want for our communities can best be achieved if we become a single trust, delivering outstanding integrated physical and mental health care.

The aim of the proposed merger is to improve our services and ensure we are providing the most appropriate and seamless information, support and treatment for people with mental health issues, physical health problems or learning disabilities. It will mean we can help people as a ‘whole person’, not just separate conditions or symptoms. As healthcare professionals in the new Trust, you will be able to ensure better outcomes for service users which we know is really important to all clinical, medical and support professionals alike. For colleagues, it will also provide more opportunity to develop and realise your potential.

While our overall name will become Gloucestershire Health and Care NHS Foundation Trust, services in Herefordshire will be given their own, separate, name, which will be Herefordshire Mental Health and Learning Disability Services.

The details of the merger will be finalised early September and the Trusts hope to be one organisation from 1st October 2019.

Both Trusts are high performing with ‘good’ CQC ratings and strong financial foundations.


Developmental Post Clinical Practitioner - (Occupational Therapist or Physiotherapist)

Band: 5

Location: Gloucester Royal Hospital / Cheltenham General Hospital

Hours: 30 hours

We have an exciting opportunity for a Developmental Band 5 Clinical Practitioner, with a wide range of experience to join us in the Integrated Assessment Team (IAT). The successful applicant will undertake roles within Gloucester Royal Hospital (GRH) and Cheltenham General Hospital (CGH ).

Integrated Assessment Team (IAT)

The IAT isa multi-disciplinary team which providestimely assessment and identification of a discharge pathway for patients presenting to the Emergency Department(ED) andAcute Medical Units (AMU’s). In addition to this, the IATteamprovide rehabilitation support to 6 Urgent Care Observational beds based within Chapel House Nursing Home.

These are 6 beds shared between IAT and Rapid Response. As an IAT clinician with a therapy background you will be able to provide therapy support for these patients, writing goals sheets and treatment plans for staff to follow.

IAT Clinical Practitioner

TheIATClinical Practitioner plays an integral role in identifying patientswho are suitable for discharge from thehigh/sub cute areas in the Acute Hospital in conjunction with ourRapid Response Service, andother community pathways.

Candidates will be expected to have a good amount of post registration experiencewithin a hospital setting in order to provide a good grounding for development.

The post holder will support local admission avoidance strategies providing patient assessment and onward referral to community based services, thus reducing unnecessary admissions to the Acute Trust andclose liaison with GHFNHST colleagues of the Frailty Assessment Service ( FAS ).

The ideal candidate willhave excellent communication skills, knowledge of admission avoidance strategies and understanding of how to access community services. You will need initiative and problem solving skills to manage sometimes challenging situations within a host environment. Full in-house supported training and an induction period will be given to the successful applicant. It would be beneficial for interesting candidates to phone for a discussion to understand the role of an IAT clinician.
