Self-Management facilitators provide a programme of interventions in the Neighbourhood Teams. The overall aim of the service is to improve patients’ knowledge, skills and confidence to manage there long term conditions and care needs.
This is a clinical role and involves care delivery as well as behavioural change. Examples of care delivery that may be delivered are:-
Insulin administration
Catheter care delivery
Simple wound management
Medication administration

The Leeds Approach is to support clinicians to have better conversations with people who live with long-term conditions, based on agreeing goals and actions. This approach underpins the Self-Management Facilitator Role by embedding personalised and patient centred care into all interventions aiming to maintain independence and working with patients and carers in order that they have the opportunity to make the most of life and remain in their home environment for as long as possible.

A programme of intervention is presently being delivered by Self-Management Facilitators working within Community Neighbourhood Teams to empower and motivate patients to manage elements of their own health care needs based on the principles of health coaching, better conversations and motivational interviewing. We are now seeking to find further suitable candidates for these posts. If you are successful in being offered a position which involves working with patients, you will be required to undertake a Disclosure and Barring Service (DBS) check. The level of check required is dependent upon the position that you have applied for. The Trust will administer the DBS check on your behalf and will recover the cost (Enhanced £48.83 or Standard £30) from your salary. There will be no charge if you are enrolled in the DBS Update Service.
