An R/N is required to join our existing team of four nurses delivering the West Somerset GP Federation’s Living Better project. This is an exciting opportunity for a nurse interested in improving patient outcomes by working on a dynamic project developed to meet the needs of a changing healthcare environment.

The post will be based at Exmoor Medical Centre and is a community-based role covering the practice’s area.

You will support patients, their families and carers to establish personalised care planning goals, encourage self-management and support independent living. You will be responsible for planning, proactively monitoring and anticipating the changing needs of patients on your caseload, co-ordinating care across all parts of the health and social care system.

You will work as part of a multi-disciplinary team supported by the GP practice, Village Agents, Community and voluntary services, secondary care and social care, amongst others.

This post will be hosted by Taunton and Somerset NHS Trust as the employer. Service delivery will be managed by the West Somerset GP Federation Living Better Team and the post-holder will report locally to our practice management team. This is a fixed-term one year post and extension of this will be subject to ongoing budget renewals.
