We are looking for a Learning Disability Nurse to join the South Norfolk Community Learning Disabilities Team, to support the nursing team Structure and the implementation of the Health Agenda. This full time Band 5 nursing post is an exciting opportunity to work in an innovative community setting.


You will be an autonomous qualified learning disabilities nurse providing assessment, treatment planning and provision for adults with learning disabilities who have a range of complex physical and psychological needs. You will be a member of the integrated multidisciplinary learning disability team in South Norfolk, working alongside a team of learning disability nurses and supervised by the clinical lead nurse. We work closely with our social care colleagues, in the same office.


You will be expected to work in isolation in a variety of settings in the community. You will gain basic managerial experience in delegating tasks where appropriate to the Healthcare Assistant in the team. You will be NMC registered and hold full and valid driving licence and have access to a vehicle.


We have a varied caseload including physical health, mental health, behaviour that may challenge others, complex health needs and comorbidities, continuing healthcare needs, psychological and emotional needs and holistic nursing assessment of wellbeing in a person with a learning disability


The South Norfolk Community Learning Disabilities Team are a group of highly motivated health and social care professionals who are committed to supporting people with learning disabilities living in the community. We include social workers, assistant practitioners, specialist nursing, occupational therapy, physiotherapy, consultant psychiatry, speech and language therapy, dietetics, psychology and integrated business support.
