Are you an experienced Healthcare Assistant and want to make the transition to community working? Do you want to be part of a dynamic and supportive team caring for people in their own homes in the Bromsgrove and Redditch area? If the answer is yes you may be interested to hear we have an opportunity for a Community Health Care Assistant based in Bromsgrove or Redditch to join our teams.

 

The Neighbourhood Teams consist of Community Nurses, Physiotherapists, Occupational Therapists, Healthcare Assistants and administration staff with GP and social care colleagues working closely alongside. The teams deliver planned nursing, therapy and admission avoidance / supported hospital discharge interventions to the adult patient population. The teams remit is to deliver a high quality service to people in their own homes, putting them at the centre of care and utilising the skills of all the multidisciplinary team where appropriate. We aim to deliver an effective, high quality service that puts patients at the centre of their own care.

 

We are looking for a highly motivated, flexible team player with a strong, quality driven patient focus and an ability to work through change to embed new ways of delivering care.

 

You should have excellent clinical, written and verbal communication skills. We provide a supportive induction programme to community work and the mentorship to help you to develop the skills and competencies expected of the role. We would consider the potential of employing the right candidate in a Band 2 development role to enable them to gain the required skills.

 

It is essential applicants have experience of working with older adults in a health/social care setting. A Level 3 qualification (e.g. A Level, NVQ3) in Care or a Level 2 qualification (e.g. GCSE Grade A-C, NVQ2) in Care and will be expected to undertake the relevant Level 3 qualification on appointment to post.

Successful candidates will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust.

The Trust offers a preceptorship year for newly qualified nurses and AHP's and attendance at preceptorship programmes is a condition of employment.

Tier 2 Certificate of Sponsorship
