We are seeking a motivated and enthusiastic people to join us and work in a fun enthusiastic committed team

The Kirklees Adult Community Learning Disability Health Team has two exciting opportunities waiting for the right people.

The team is a multi-disciplinary team that provides specialist health support to both individuals, and their wider support networks. The team work agilely and covers the whole of the Kirklees geographical area. The administrative base is in Dewsbury, but you will be out and about in people’s homes, workplaces and services across Dewsbury and Huddersfield

Post One is In The Intensive Support part of the Team (IST) which works proactively and intensively with a small number of clients, aiming to reduce hospital admissions and placement breakdowns. Interest and/or experience in working within a positive behavioral support model. This post is full time 37.5 hours

In this post you will work with the nurses to offer assessments and interventions to help people with a learning disability improve their physical and mental wellbeing. This may include some community health promotion activities and events

For both posts we are looking for candidates who demonstrate a positive attitude to working with people with learning disabilities and a commitment to promoting a high quality of life for people with learning disabilities. Experience in working with people with learning disabilities and/or on the Autistic Spectrum would be an advantage and experience of working in care provision is essential.
