Title of post: Healthy Minds Practitioner (Chapeltown - Extended Access Service)

Northpoint Wellbeing is a charity funded by the NHS that provides therapeutic services across the region.

We are recruiting for the post of Healthy Minds Practitioner to join the Chapeltown Mental Health Extended Access Service responsible for the delivery of mental health triage, symptom management and care navigation out of normal working hours to clients in the Chapeltown area of Leeds. Supported by Primary Care, the Extended Access Service provides cover up to 8pm on weekday evenings and between 9am-12pm on Saturday mornings. The post is based within Primary Care and managed by Northpoint Wellbeing.

Predominantly working remotely (home based or equivalent) the post-holder will provide ‘extended access’ telephone triage, assessment and decision-making support to ensure that patients with common mental health needs gain access to the most appropriate pathway and intervention. There will be a requirement to deliver some face-to-face support (one day per week, TBC) in at least one of three GP practices connected to the service (Woodhouse Health Centre, The Reginald Centre or Chapel Allerton, TBC). The role combines the skills of triage/assessment, care navigation and psychological formulation.

Salary: £24,214 - £30,112 pro rata (salary range is equivalent to Band 5 on NHS Agenda for Change).

This is a part-time post (24 hours per week) offered as a 12 month fixed-term contract with the possibility of an extension subject to further funding. We are looking to recruit one 24-hour post but may consider two 12-hour posts.

Hours: there will be a requirement to work until 8pm on weekdays (2pm-8pm) and Saturday mornings (9am-12pm) exact times TBC.
