We have a range of exciting opportunities within the Community Specialist Palliative Care service. The service provides rapid response home visiting within 24 hours, to those patients triaged as urgent. The Community Teams work across three boroughs; City and Hackney, Newham and Tower Hamlets. We work closely with our colleagues in community and primary care to help deliver integrated and partnership working, through joint working, education and training.
