Southwark Clinical Commissioning Group has an exciting opportunity for a Continuing Healthcare Clinical Commissioner to join our Nursing and Quality Team with a focus on Adult and children eligible for continuing healthcare funding.

The team work with clinicians from primary care, community services, acute trust and other key stakeholders to assess eligibility for NHS Continuing Healthcare funding, plan, review, and redesign high quality, cost-effective care in order to improve patient outcomes and reduce health inequalities. The team work jointly with Southwark Local Authority to ensure the healthcare and education systems are aligned to provide the best quality care for local families.

Perhaps you have worked in continuing healthcare team as a nurse assessor, as a discharge coordinator or you are an experienced community or district nurse. You possess excellent clinical knowledge; you have an ability to solve problems and a team player. We want to hear from nurses who are passionate about improving services for adults and children. Whether you have commissioned services before is not important, it’s actually what you do now, day to day that matters to us. If you have experience in managing complex cases and a sound clinical background we would like to hear from you. This role will give you the opportunity to work as part of a multi-professional team across health, social care, community health services and the acute sector with a clear patient focus.
