The Role


The Hackney and City Transformation programme is charged with delivering ambitious new models of care across primary care, secondary care (including mental health), community health and social care in City and Hackney so that truly integrated, effective and financially sustainable systems can be provided that meet the population’s health and wellbeing needs.

 

Technology plays a crucial role in delivering the capability required to support these new models of care for health and social care serving a population of ~250,000.

 

The postholder will play a key role in transforming the way health and social care services are provided across City and Hackney. Drawing on their ability to think innovatively, problem solves and learn they will develop effective business solutions that make use of the latest digital technology.

 

The post holder will use their excellent project management skills, strong organisational and influencing skills to secure buy-in to the digital agenda at a senior level.

 

As the Digital Transformation and Implementation Lead (Planned Care) you will be responsible for:

 

  • Supporting the delivery of the Transforming Outpatient Services programme by making services more responsive to the needs of patients and staff through the use of technology. This will include the development of virtual referral reviews, an online self-referral platform and the digitisation of letters and DNA processes.

 

  • Supporting the integration and efficient management of services in relation to Continuing Healthcare including reviewing and developing the functionality of the resource allocation and case management systems, assisting with the role out of Personal Health Budgets, and developing the platform for a Dynamic Purchasing System for care packages.

 

  • Improving the CCG clinical website content to facilitate information sharing and referrals between services.

 

  • Identifying opportunities and facilitating the delivery of Planned Care workstream priorities through the use of digital technology and a robust project management approach.

 

  • Supporting the development of an overarching digital strategy and programme plan for the IT enabler work stream, including timetabling key milestones, events and decision making points to ensure key IT projects are firmly established.
