ELFT (East London NHS Foundation Trust) is making a positive difference to thousands of people’s lives, providing support so that they can fulfil their potential and enjoy healthy lives.

The Telehealth team consists of Telehealth Care Support Workers and Clinical Leads and nurse who work with patients diagnosed with a long term medical condition to help them monitor, manage and control their own health. Long term conditions include high blood pressure, diabetes, breathing problems (chronic obstructive pulmonary disease or COPD) and heart problems that need to be managed carefully to avoid periods of illness and unnecessary hospital visits and admissions. Telehealth simply means providing health care at a distance. In Newham, we are leading the way in using Telehealth technology to support people to help manage their condition, maintain their health and well-being and live independently. ELFT Telehealth team is the first in the country to have achieved the EU Telehealth code of practice accreditation.


ELFT Telehealth has won an innovation award (2016) in the health tech world to do a project in mental health. ELFT Telehealth has also been shortlisted for the nursing times award 2016 for integrated Approaches to Care category.
ELFT offers three levels of Telehealth service, namely High level vital sign monitoring, Florence (Simple Telehealth using mobile phones) and Guide, delivered within its step up/step down model of integrated care. The integrated team works closely with Rapid response and extended primary care services i.e. community services and operates a step up and step down model and provides an excellent high quality care to the service users and staff.
The post holder would ensure that all service users are regularly assessed, monitored and reviewed and have a strong knowledge and understanding and management of Long term conditions. The post holder would have good clinical reasoning skills and would be working closely within the teams, GPs, consultants, specialist services and other stake holders.
The post holder should have a strong understanding and knowledge of Telehealth and its application in ehealth and excellent in IT and communication skills.


The post holder would have clinical work load in the extended primary care service alongside Telehealth and provide advice to the MDT as needed and also take part in audits, workshops etc.
The post holder will be expected to deputise at times demonstrating leadership and organisational skills and seek advice of seniors where necessary and facilitate smooth running of the service


The post holder would need to have excellent communication and interpersonal,leadership skills and articulate their knowledge on the Telehealth in conversations with other colleagues and other stakeholders to facilitate development of the service.
