An exciting position has arisen for an experienced full time Diabetes Specialist Nurse on a permanent contract. Previous diabetes nursing experience at band 6 level is essential for this post which offers a unique opportunity for the successful candidate to work both in the community and acute settings.

The diabetes nursing service at County Durham and Darlington Foundation Trust provides specialist care and advice for people with diabetes in both the inpatient and outpatient settings. Other specialist Diabetes services include pre-conceptual care, antenatal care, insulin pump therapy, transitional care, adolescent care and specialist renal clinics.

The post will also support the County Durham & Darlington Diabetes Model of Care; this involves the delivery of diabetes clinics in GP surgeries and the upskilling of primary care staff across 6 localities.

The post holder will need to be experienced in adult nursing, able to work autonomously within a multidisciplinary team and be capable of making informed clinical decisions, often in complex situations. A key feature of the role is to educate, empower and support, not only people with diabetes, but also health care professionals involved with patients who have diabetes, in both primary and secondary care. They will be required to work alongside the Lead DSN developing specialist service provision including clinical leadership for other members of the health care team.

It is anticipated that the successful candidate will cover all aspects of diabetes care within the team. Following induction the post holder will have a job plan based around service requirement.
