The Health Case Management Service provides a case management function for the population of Leeds (18 years and above) who are eligible for NHS Fast Track and Continuing Healthcare funding; this includes hospital and care within their own home, over 7 days, Monday to Friday 08:00 to 20:00 and Saturday and Sunday 09:00 to 17:00.

The Health Case Management Service is a citywide service, divided into two areas (North and South). The teams consist of Health Case Managers, Health Case Management Co-ordinators, Health Case Management Assistants and Administrators who are professionals with health and social care backgrounds and have a wealth of experience in all aspects of Health Case Management.

The Band 2 administrator apprenticeship opportunity is welcoming candidates who exhibit the values and behaviours of Leeds Community Healthcare and can demonstrate attributes required for the role; IT literate and have good communication skills. We are looking for people who are passionate about providing high quality customer service for patients and colleagues. Your role will include a variety of administration duties such as being front of office, the management of referrals and tasks, inputting and retrieving information from SystmOne (training will be provided), monitoring and ordering of stock and handling incoming and outgoing mail. Your core working hours will be from 08:00 to 16:00.

You will be supported and report to band 3 administrators and line management of a Clinical Quality and Operational Lead; this will be through regular sup'ervision and the appraisal process which includes continuous professional development.

The level of check required is dependent upon the position that you have applied for. The Trust will administer the DBS check on your behalf and will recover the cost from your salary. There will be no charge if you are enrolled in the DBS Update Service.
