An exciting opportunity has arisen for a CYP Continuing Care Nurse Assessor and Case Manager.
We are looking for a committed and enthusiastic individual to join the team at Sutton CCG.

 

The postholder will support in the delivery of Children’s Continuing Care, working closely with CYP and their families, Local Authority partners, and care providers to oversee assessment, decision-making and development of packages of care in accordance with the requirements of the National Framework for Children Continuing Care.

 

There will be a clear focus on working with CYP and their families to maximise opportunities for long-term outcomes and wellbeing. Additionally the postholder will work with the wider commissioning teams, key stakeholders and providers to ensure that standards of care for all individuals are delivered in accordance with their contracts and service specifications.

 

The postholder will be an excellent communicator with a good IT ability and administrative skills. You will have experience of working in a clinical MDT and be competent and confident in the delivery of accurate and timely information, including the preparation of reports in respect of outcomes and delivery.

 

The successful candidate will be a Registered Nurse (child), Evidence of up to date clinical practice and experience of providing high standards of care will be required.
