From the east to the west, under impressive skies, the varied landscape of Suffolk changes subtly.

Stretching north from Ipswich and Felixstowe, the Suffolk Coast, (with five sailing havens), and heaths has miles of Heritage Coast set in an Area of Outstanding Natural Beauty with beautiful beaches, stunning countryside and charming market towns for you to discover. The Ipswich waterfront is a beautiful ‘jewel in the crown’ and seamlessly blends moored yachts, the iconic university building, a working port, modern apartments and the alongside a lively mix of retail, bars and restaurants. Beyond this, Ipswich has three theatres, a thriving retail area and an abundance of independent boutiques and eateries in some of the most historic and architectural buildings in Suffolk.

Inland are undulating fields, enfolding picturesque villages and slow-flowing streams. Some of the finest ancient woodlands can be found in this part of Suffolk, an area that abounds with ancient market towns, including Bury St Edmunds and Lavenham hardly changed since its heyday as an important Middle Ages wool town.

Join our fantastic expanding team… NHS Ipswich & East Suffolk and West Suffolk CCGs have exciting opportunities for Registered Nurses (Adult, Mental Health or Learning Disability) to both coordinate NHS Continuing Healthcare assessments, case manage individuals who are eligible for NHS Continuing Healthcare, and support the newly implemented Discharge to Assess model. We are a forward thinking, motivated, friendly and award winning team, and are looking for an equally innovative, forward thinking, enthusiastic and confident nurses who would like to join us. We have a number of roles available within the NHS Continuing Healthcare team which offer varied and interesting work, including within our assessment teams where you will be involved in undertaking NHS Continuing Healthcare assessments for people at home or in care homes, as well as within our Personal Health Budget team, where you will be involved in the case management of individuals who are eligible for NHS Continuing Healthcare and wish to manage their own care package. We enjoy a strong working relationship with our local authority colleagues who you will work with regularly. You will work closely with colleagues in other organisations such as the acute hospitals, and community services and build professional relationships to improve patient experience. These roles are challenging but rewarding and offer the opportunity to rotate within the team to further develop skills and knowledge regarding NHS Continuing Healthcare. Our team is warm, friendly and supportive and our commitment to you will involve a full induction, opportunities for professional development and support with career progression and talent recognition. You are required to be a car driver and have access to a car as you will regularly be assessing individuals across the county of Suffolk and occasionally out of county. As an award winning service there is a focus on continuous improvement to ensure we deliver high standards and work productively to ensure robust and timely decision making for our patients. Previous experience of NHS Continuing Healthcare is not essential however, you should have experience in assessing individuals with complex health and social care needs, including coordinating and planning care. You will be an excellent communicator with sound interpersonal skills. You will be expected to have good IT skills, including Microsoft Word and be experienced in using a clinical system for recording patient information. You will need to have the ability to manage emotive and sensitive issues with tact and diplomacy, work well under pressure, negotiate with confidence and maintain a high level of professionalism and productivity.

Probationary Period

All positions within the CCG are subject to a 6-month probationary period; with the exception of those candidates with continuous NHS service.
