Would you take great satisfaction tailoring interventions and delivering high quality care tovulnerable and complex young people? Lead our Primary Care Team to achieve the best health and wellbeing outcome through the use of tailored interventions.

We're currently looking for a Primary Care Team leader to complement our award winning Primary Healthcare Team. As a passionate and committed registered nurse, you'll lead the team to ensure young people's health needs are met when they return to community settings or transfer to another secure environment. You will have proven leadership skills, with the ability to make clinical decisions autonomously within a fast paced changing environment.

The Role

No two days are ever the same. As a Primary Care Team Leader you will be responsible for leading the team on a daily basis in a hands on clinical role. The team are wing based; each Team Leader has their own wing which requires case management. You will also facilitate nurse led clinics, administer medication, immunisations and vaccinations, triage and treat minor illness / injures, attending emergency situations and delivering age appropriate health promotion sessions to this complex, yet rewarding client group.

The primary role is based at HM Wetherby Young Offenders, with cross cover at Adel Beck Secure Children's Home.

 

Our Service

HMP Wetherby Young Offenders Institute (WYOI) is a secure male young person centre housing 336 young people aged between 15 and 18 years. Leeds Community Healthcare (LCH) provide a 24/7 Service to the young people within the establishment.

We are also commissioned to provide care to the young people at Adel Beck Secure Children's Home; this is a 24 bedded unit (male and female) age 10 to 17 years.

LCH have been providing healthcare here for over 10 years, meaning we have a unique understanding of children and young people's needs and the associated challenges and opportunities these bring.

HMP WYOI has excellent onsite facilities, with free parking, two staff gyms and a coffee shop. It is situated very close to the A1(M) with easy links to Leeds, Harrogate and York. Wetherby is a busy market town which boasts specialist craft shops, quaint tearooms and restaurants. It makes a wonderful, central base for days out to the Yorkshire Dales, the North Yorkshire Moors and the East Coast.

 

About You

You will be a dynamic, positive clinical leader with good organisational skills. You will be creative and supportive, assisting in managing change within the service at a clinical and operational level. You will provide operational management to the primary care ream, working alongside the current team leaders and Primary Care Manager to provide the young people in our care with a comprehensive and high quality health service. Experience of working within a secure environment is desirable. A comprehensive induction package is provided.
