Band 7 (full-time) Admiral Nurse - Acute

Wye Valley NHS Trust, Herefordshire

37.5 hours per week

As part of an exciting new service development, Wye Valley NHS Trust is working in partnership with the charity Dementia UK to provide an Admiral Nursing service, supporting families affected by dementia across the acute-community pathway.

We are looking for an exceptional, registered (adult / mental health) nurse with a passion for and experience of, providing person-centred and relationship-centred dementia care. Combining direct work with families with supporting the best practice of health and social care professionals, you will call upon your impressive knowledge of evidence-based bio-psychosocial interventions to improve the experience of carers / families affected by dementia.

The Admiral Nurse will be based in the County hospital and will focus on leading and delivering person-centred dementia care and optimising the outcomes for people with dementia and their carers within the hospital setting, and will undertake line management for the Band 6 community Admiral Nurses who offer an outreach into the community and support families with complex needs at high risk of admission. The Admiral Nurses will work together with a cross-system mindset to ensure that care transitions are well managed, discharge is supportive and effective and carers feel better able to carry on in their caring role.

Dementia UK will support you to maintain and strengthen your clinical expertise as a specialist nurse through monthly clinical supervision and practice and professional development. You will become part of a network of Admiral Nurses, and will have the opportunity to access relevant post-graduate courses and develop professionally, informed by the Admiral Nurse Competency Framework.
