Community Heart Failure Specialist Nurse

Band 7

 

An exciting opportunity has arisen within Warrington and Halton Cardiac Therapy Services, for a full-time Community Heart Failure Nurse to join our Team.

 

Based at Health Service at Wolves, Warrington, the post holder is also required to work in both a hospital and community setting and will work within a multidisciplinary team.

 

The post holder will support other community health care professionals, to ensure that patients receive optimal care in keeping with evidence based practice and ensure that patients and carers receive appropriate treatment, and support.

 

The role will include care of patients discharged with heart failure within a community setting, home visits and within a clinic setting at Warrington Hospital. The post holder will also be required to assist with the heart failure cardiac rehabilitation programme. The role requires assessing and case managing patients with heart failure, home visits, clinics and exercise assessment and classes.

The post holder will also be required to optimise heart failure medications as per NICE guidelines and WHH Non-medical prescribing policy.

 

Applicants should have a masters or equivalent qualification. Alternatively, applicants with a commitment to gaining a masters qualification may be considered if they meet other essential criteria. Both full-time and part-time applications will be considered.

 

Warrington and Halton Cardiac Services are currently undergoing service redesign, this is a unique opportunity to be involved in service development.

All posts are subject to NHS Employment Check Standards including a DBS check, if applicable. Depending on the classification of the post, in-line with DBS guidelines, a Standard or Enhanced disclosure will be required which may include a check of the ‘Vulnerable Adults List’, the ‘Children’s List’ and/or the Independent Safeguarding Authorities (ISA’s) barred lists.

A charge of £39 (Standard) or £60 (Enhanced) will be made by the successful candidate which will be deducted from the first 3 months’ salary. In addition to this, you will also be required to re-pay any future costs that may arise in relation to DBS fees.

NB. Current employees of Warrington and Halton Hospitals NHS Foundation Trust who have had a satisfactory DBS check, carried out by this Trust during the last 3 years, will not be required to undertake a new one. Where staff have not had a check (classification appropriate to the post offered) within the last 3 years, they will be required to undertake and pay for this, as per the arrangements above.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
