NHS Continuing and Complex Healthcare Nurse Assessor – Band 6

An opportunity has arisen within NHS Stockport Clinical Commissioning Group (CCG), Continuing & Complex Healthcare Services nursing team for a Nurse Assessor (22.5 hours/ 0.6 WTE).

NHS Stockport CCG is seeking to recruit a highly motivated, enthusiastic and forward thinking nurse to work as part of a professional team. The successful candidate will need to be able to demonstrate knowledge and skills in person-centred assessments, and the ability to work within a multi-agency environment. All nursing qualifications will be considered ie; RN, RMN, RNLD.

If you are a good leader, understand change management and have excellent communication skills supporting the philosophy for delivering high quality services to people, this would be ideal opportunity for you.

The successful applicant will be required to work autonomously and collaboratively with a well-established skill-mixed team and support the new ways of working outlined by the Department of Health National Framework for NHS Continuing Health Care and NHS-funded Nursing Care.
