10PA Consultant in Palliative Care

Applications are invited for a Consultant in Palliative Care post (10PAs) at The Great Western Hospitals NHS Foundation Trust (GWH) in Swindon.  

 The Trust serves a population of 350,000 and is part of the Thames Valley Cancer Network and the South West Health Science Network. At the last Care Quality Commission (CQC) End of Life Inspection the Trust received a rating of “Good” for end of life care. The team also received an award from Health Education South-West for delivering education to support personalised care planning for the dying.

 Patients seen by the hospital palliative care team are often within the last year of their lives and have conditions such as respiratory disease, heart failure, dementia, renal failure and cancer. The team provide specialist palliative care advice to inpatients and families and act as a specialist resource to clinical teams. The team have developed advance care planning resources used across acute, community and hospice services. Innovative practice to support the Trust in delivering excellence in end of life care is required meeting national strategy and guidance.

 We are looking for a motivated consultant to deliver:

 

Leadership in the implementation of nationally directed principles and guidance

leading to:

  • Improved End of Life decision making and improved delivery of high quality patient centred care.
  • Necessary specialist contribution to the implementation and review of a comprehensive End of Life Strategy for the Trust. 

 Compliance with Cancer Peer Review Measures 

  • Specialist Palliative Care
  • Cancer of Unknown Primary
  • Upper GI
  • Lung

 Visible and responsive Consultant led clinical activity  

  • Ward based activity 
  • Outpatient activity, initially for advance care planning but with the possibility of expansion, for example for patients with non malignant conditions such as renal impairment and neurological disease 
  • Enabling Trust participation in research activity 

Attendance at meetings incorporated in job plan and completion of associated actions 

  • End of Life Committee
  • Mortality Meeting
  • Resuscitation Committee / TEP working group
  • Cancer Management Meeting / Cancer Clinical Leads where relevant
  • Acute Oncology Working group
  • Cancer Working group
  • Swindon and Wiltshire EOL steering groups

 Educational activity, examples include: 

  • Educational support to CNS team
  • Teaching to foundation and CMT doctors
  • Departmental teaching eg to DOME / ED
  • Presentation at Grand rounds
  • Development and delivery of mandatory e-learning training supporting care of the dying

 Palliative care specialist nursing is provided by Prospect Hospice In-reach team Monday- Friday 9am-5pm. An out of hours telephone service is provided by Prospect Hospice. The team work closely with community services and the hospice to ensure patient preferences in care are supported. The post-holder will be working in the acute Trust but liaise closely with colleagues at Prospect Hospice.

The successful candidate will participate in both undergraduate and postgraduate teaching across the Trust. 1PA is specifically supporting undergraduate education funded by Swindon Academy.

The post is full time (10PAs) and would be suitable for someone wishing to further develop their skills in Palliative medicine or embedding their skills as a palliative care consultant.

There will be appropriate opportunities for training and time for clinical audit and continuing professional development.

 Applicants should have MRCP and it is desirably to hold entry on Specialist Register for Palliative Medicine or within 6 months of obtaining CCST/CCT. Full GMC registration is essential.

 The Great Western Hospital opened in 2002 with well-appointed accommodation for staff and patients. The hospital is a Teaching Hospital for undergraduate students from both Oxford and Bristol Universities. The Trust provides acute services to Swindon and the surrounding areas (catchment 350,000) and community services to Wiltshire (catchment 750,000).

Swindon is an expanding town, which sits adjacent to the M4, at the heart of Southern Central England in the Wiltshire countryside. The Marlborough Downs, the Cotswolds and the towns of Oxford, Bath and Bristol are all within easy reach.

All candidates are required to pay for their own DBS check where applicable. Standard DBS check costs £26 and will be recovered from your first pay; Enhanced DBS check costs £44 and will be split and recovered over the first 2 pay.

Bank posts requiring a DBS check; payment must be made prior to commencing.

Apprentices are exempt from payment.
