SENIOR MENTAL HEALTH PRACTITIONER

Mental Health Services in North East Lincolnshire are delivered by NAViGO – a not-for-profit Social Enterprise, established in April 2011. We provide a unique health and social care service to a population of 158,000 improving the quality of life for those experiencing mental health difficulties, their carer’s and families and helping people to remain as independent as possible within the community. In its first few months, NAViGO won two prestigious national awards – the Guardian Public Service Award 2011 and also the Service Delivery (Complex needs) category. Within Older People’s Mental Health Services there is a vacancy for a Senior Mental Health Practitioner (RNMH, Social Worker or Occupational Therapist) within Older Peoples Community Mental Health and Memory Service. This would be an opportunity to work within a memory service that has twice been awarded the “excellent” status through accreditation with the Royal College of Psychiatrists.

 

The post holder, based within CMHMS, will act as care co-ordinator within the CPA process for service users with complex behavioural and psychological symptoms associated with Alzheimer’s disease/mixed pathology dementia or otherwise suitable for treatment with ACI and other memory enhancement medication.

 

 

The post holder will be expected to work flexibly and creatively and will provide assessment, care planning and on-going interventions to older people in conjunction with other services. The post holder will have current and relevant experience of working within a Mental Health setting for Older People and will have knowledge of relapse and risk management prevention and have the ability to manage change and work within a changing environment. Senior practitioners within the service will provide supervision and role development will be actively encouraged.

 

The post holder will be committed to the principles of person centred care and be able to teach and role model to others. They will be imaginative and broad in their outlook to care provision. They must be able to work upon their own initiative, whilst being committed to the principles and practice of effective team working.

 

A full driving licence is required as is access to a vehicle for works purpose.
