The Sirona Community Rehabilitation Team (CRT) is an innovative and dynamic service. We aim to assist in preventing hospital admission and to facilitate timely hospital discharge if an admission to the acute trust has been unavoidable (Discharge to Assess). We also provide Planned (Domiciliary) therapy to ensure that patients are assessed and supported early in the journey to maintain their optimal level of independence and hence avoid longer term concerns. We provide active rehabilitation to patients in their place of residence to maximise their functional potential and support them to remain independent within their homes.

We are seeking enthusiastic and motivated Occupational Therapists to join our busy and dynamic team. We are looking for experience of community working, to promote independence and choice for our service users and their carers. The posts are full time and you would be based either at Patchway Clinic or Downend Clinic working across the Severnvale & Kingswood Localities

In return, we will give you the opportunity to develop specialist community skills and enhance your multidisciplinary working; we will provide you with regular training and supervision opportunities as well as the opportunity to develop your leadership skills in supervising Rehab Support Workers.

You will be working within a friendly and supportive multidisciplinary team alongside other Occupational Therapists, Physiotherapists, Speech and Language Therapists, Rehabilitation Support Workers and Physiotherapy Technicians. We collaborate closely with our Community Nursing colleagues and other Acute, Community and Social Care partners. We offer a flexible service which covers extended hours operating 7 days a week, to support our service user needs.

Working arrangements include weekends and bank holidays on a rota basis.
