High Weald Lewes Havens is a relatively small CCG (population c170,000) that has a strong track record of innovative working and delivering real improvements to patient experience.

High Weald Lewes and Havens CCG are looking for an experienced Senior Pharmacist to join our well-established Medicines Management team. This is a maternity cover within an established Clinical Commissioning Group for individuals who are interested in delivering real, demonstrable, transformational change as part of medicines optimisation that will seek to improve patient outcomes for our population.

We are transforming the way we work with our practices to a far more integrated model working as part of the multidisciplinary GP practice team. This gives the pharmacy team the opportunity to work right at the front line of primary care services, addressing interface issues, using your clinical skills to tackle medicines problems and offering rewarding patient facing roles such as medicines optimisation clinics.

You will need to be an enthusiastic individual who is driven by a passion for high quality services for our patients and will support the leadership of the team to deliver the medicines optimisation vision and strategy. This is a role for someone with the leadership skills and experience to champion the Medicines Optimisation agenda across the health economy.

Ideally, we are looking for someone with excellent clinical knowledge and proven relationship management skills across health and social care interfaces. Applicants should have excellent communication skills and experience of working in multidisciplinary teams in the primary care environment would be preferable. Demonstrable experience of change management, project management and protocol development would be welcomed. Good IT skills are necessary and a working knowledge of GP computer systems would be an advantage. Access to own transport is required. In addition, you should have experience in the education of healthcare professionals and team development.

The role offers you the chance to help lead the Medicines Optimisation agenda, working alongside a highly committed medicines management team with ambitious plans for transforming the way we optimise medicines use for people in High Weald, Lewes and Havens.
