Acute Hospital at Home Service - DCHFT

Physiotherapist Band 5 with progression to Band 6

Reports to: Team Lead/Service Manager/Matron /Head of Therapies

An opportunity has arisen to work in an exciting integrated model for the delivery of patient care in Dorset County Hospital’s Acute Hospital at Home service. This service proactively manages and treats a defined caseload of acute medical & rehab patients, who have complex care needs in the short term, on a multi-disciplinary basis in their usual residence.

Car Driver Essential.

Patients are offered continuity of care by following them from hospital into the community and the post provides the opportunity to work more closely with community and primary care colleagues in a seamless service. We are looking for a dynamic Physiotherapist with some rotational experience, to work in our multi-disciplinary team, ensuring a patient focused and individualised approach. Ideally you will have acute and community experience and have experience of effective multi-disciplinary team working in its widest sense

This is a service with the patient’s needs at its heart and aims to support admission avoidance and early discharge. The focus of the service is the frail elderly which is important in Dorset given that 29% of the population is of retirement age

The Service is based in the hospital at Dorchester, the traditional county town of West Dorset, made famous by the novels of Thomas Hardy. The town has a quiet centre, although with the influx of people relocating from London, Essex, the Midlands and Home Counties, new shops and restaurants have opened adding to the mix of modern and traditional.

West Dorset with its outstanding scenery and rugged coastline is a most enjoyable part of the country, characterised by a number of small towns, each with a distinct identity.

Whatever your outdoor pursuit you can find it here, for example: hiking, wall climbing, horse riding or water sports such as sailing, surfing, kayaking ,diving and so much more! You will find excellent transport links to London & Bath.

The hospital is close to the town and is on one site. It is easily accessed by bus and rail networks and there is parking on site.
