Wiltshire CCG would like to receive applications for these exciting roles within the Continuing Healthcare Department.

We are in the process of transforming the department and have two Band 7 roles which will be pivotal in shaping the future of the department.

In addition to team and caseload management the role incorporates a specialist element to enhance the development of the department.

 

  1. Individual Commissioning Co-ordinator – Key Stakeholder Liaison

In conjunction with the Operational Lead(s) the post holder will provide specialist leadership and develop and maintain an understanding and overview of:

  • Develop and maintain a liaison role with Wiltshire County Council Adult Social Care

  • Develop, define and maintain the liaison role for Care Homes, NHS providers domiciliary care providers, hospices and GP Practices providing a communication conduit for the CHC Department.

  • To be the recipient of feedback from the above organisations and provide support and resolution with support from the Operational Lead

  • To develop and maintain a specialist interest in Learning Disabilities.

The Specialist Lead element of the role required the post holder to engage in effective and collaborative working with colleagues throughout the CHC Team advising and supporting practice. It is anticipated that the specialist lead element of the role will represent 0.5 WTE of the role.

In addition to this the post holder will be:

  • Accountable for a delegated group of individuals who are CHC Funded and have complex needs.

  • Share responsibility with Individual Commissioning Co-ordinator colleagues for developing and delivering the PHB Policy and process throughout the team. Implementation of departmental Audit policy and process

  • All statutory requirements arising from Safeguarding Adults (SA) Policy, the Mental Capacity Act (MCA) and the Deprivation of Liberty Safeguards (DOLS) and ensure that Wiltshire CCG is compliant with these.

  • To develop a working understanding and knowledge base of the needs of Individuals with Learning Disabilities and enduring Mental Health needs

 

 

  1. Individual Commissioning Co-ordinator - Operational Performance

    In conjunction with the Operational Lead(s) the post holder will provide specialist leadership and develop and maintain an understanding and overview of:

  • The requirements for Fast Track referrals received by the department as well as work with the Integrated Governance Lead to assure regular audit of Fast Track referrals and feedback through the departmental Integrated Governance Committee.

  • The FNC review process ensuring that it is in line with the National Framework for Continuing Healthcare 2018

  • The 28 Day performance target for all new CHC applications and the review of needs for patients who have existing Continuing Healthcare eligibility.

  • The internal and NHS England reporting requirements for Continuing Healthcare and support the CHC Business Manager in delivering the reporting requirements.
