The South Cotswolds frailty service is seeking to recruit a dynamic and motivated nurse with advanced clinical and communication skills to join the team supporting care for frail people in the South Cotswold Locality. The role of the Community Matron is to work as an autonomous nurse at an advanced level, making complex clinical decisions, and using expert clinical judgement to improve clinical and quality of life outcomes for individuals with complex health conditions

The South Cotswolds Frailty Team is employed by the South Cotswolds GP practices and the post holder will be linked to identified GP practices. The successful applicant will also forge close links with the local Integrated Community Team, social prescribers, community hospital, acute sector colleagues, voluntary sector and other community resources.

The post holder will be responsible for the line management, supervision, training, support and leadership of a Well-Being Coordinator within the team. You will have the opportunity to lead on areas of service development and deputise for, and undertake work delegated by the Senior Community Matron as required.

This is a permanent role, and the hours of work will be between 8am and 6pm, although in the future expansion of hours may be considered to include 7 day working
