Band 7 Advanced Practitioner, Dementia Diagnostic Assessment

The Bristol Dementia Well being Service is a joint venture between Alzheimer’s Society and Devon Partnership Trust. It provides Dementia services to people who are registered to a Bristol GP. We are located across three community sites and support the people of Bristol with Dementia to live as well as possible, for as long as possible, in their own community.

We have an opportunity for an experienced clinician to join the team to as a Band 7 Advanced Practitioner who specialises in Diagnostic Assessment. This role will provide clinical support, advice and guidance citywide.Core working hours are Monday to Friday 09.00-17.00.


The post holder will require:

  • robust clinical triage skills which they will use as a core part of the role as well as contributing to the multidisciplinary triage

  • awareness and understanding of possible complicating or excluding factors to assessment and diagnosis

  • a background in diagnostic assessment and formulation which they will use both for their own clinical work and to enable others to formulate a diagnosis based on their findings

  • experience of people with a range of needs including Mental Health diagnoses

  • an awareness of and willingness to work closely with other relevant parties such as neurology

  • an ability to provide clinical supervision, education and training which will be used to support practitioners and primary care colleagues such as GPs, among others

The role includes independent clinical activity holding a case load of complex diagnostic assessment and citywide consultative work. This will include clinical support and supervision in both group setting by way of diagnostic formulation meetings and on a 1:1 basis. There will be a focus on education across the city by way of joint visits with practitioners to complex diagnostic assessments and/or feedback visits and attending GP surgery meetings.

The post holder will work closely with the medical team and psychology team within the service and have access to clinical supervision.

This service provides all staff with opportunities for excellent training and development with the potential to further develop skills in cognitive assessments in conjunction with the psychology team. Not least to make a significant positive difference to the lives of people living with dementia.
