We are a high performing and successful multidisciplinary team delivering high quality person centred specialist assessment, treatments and care.

Our team works with people aged 65 and over (younger in some cases) specialising in both functional and complex organic conditions needing secondary service interventions, serving the Stroud, Berkley Vale, Cirencester and South Cotswolds locality.

We work within a CPA and Care Management framework to specific service specifications with defined quality and performance indicators. Our service users rely on us working in a highly integrated and collaborative union with an array of other teams including: Memory Services, Crisis and Home Treatment, Hospital Liaison, Mental Health Inpatients, Social Services and localised Integrated Care teams; enabling us to deliver effective seamless responsive care, reflective of the service user’s needs at any time whilst in our service.

Applicants will have:

  • Post registration community experience within mental health services.

  • Be self-motivated, well organised and committed to the delivery and development of high quality care.

  • Excellent communication and problem solving skills, flexible and creative approach.

  • Proven ability to effectively manage and care coordinate a complex caseload, reflecting all components of care management policy and CPA.

  • Ability to lead and effectively assist in managing team workload, supporting junior staff through a coaching approach, supervision and appraisal.

  • Able to work autonomously and as part of a team (will be subject to lone working).

  • Show evidence of continuous professional development and contemporary evidenced based practices.

  • Able to transport self around the locality and undertake statutory and mandatory training.

  • Good knowledge / working experience of MHA, MCA, DOLs, and Safeguarding.

This post requires the post holder to be able to undertake all statutory and mandatory trainingand post holders must be able to undertake physical interventions. Fitness for physical intervention will be assessed by Occupational Health.

Applicants should note that all appointments to 2gether NHS Foundation Trust (excluding Medical Staffing appointments) are subject to the completion of a satisfactory 6 month probation period.

Please be aware that all employees of the Trust who need to have a DBS check in order to carry out their role, will be required to repay the cost, which is currently £50 for an enhanced check or £32 for a standard check.

As part of the Trust requirement to practice safely all internal applicants, including bank staff, are required to be up to date with statutory and mandatory training, or have booked dates for expired training.
