We provide a large number of mental and physical health services. Poor physical and mental health are linked and our ambition is for people to have their physical and mental health problems identified, assessed and treated at the same time.

We have exciting opportunity for a Community Mental Health Nurse to provide innovative and effective support and interventions within the service. We work within a Clinical Pathways’ model and this role is placed within our Personality Disorder Team. The Personality Disorder Team has been established for five years and we are keen to develop the service further.


You will be working within a supportive Multi-Disciplinary Team friendly established integrated team including your social care colleagues to provide a service to a wide range of people with mental illness. This will be an excellent opportunity for you challenge yourself as a Practitioner.


The Role


We are looking for a Practitioner with an enthusiasm for working with people with challenging and complex needs.


-To have a willingness to take positive risks with Multi-Disciplinary Team support.


-To create collaborative and realistic goals with patients.


-To have a robust approach towards Care planning and Crisis Support.


-To be a creative and dynamic member within the MDT.


-You will be working under the Care Programme Approach, ensuring this is effectively delivered. This will also include undertaking social care assessments under the Care Act.


What We Offer:


-Group Supervision with a Service User Consultant as part of the Co-Production model. We are developing a group under the Managed Clinical Network which involves a more structured timely focus with patients.


-1:1Supervision with the Senior Practitioner for the Personality Disorder Pathway.


-Weekly Meetings within a Multi-Disciplinary Team inclusive of a Psychiatrist, Nurse, Social Workers, Psychologists, Occupational Therapists, Vocational Services and Peer Support.


-Support through a Complex Case Forum weekly with Psychotherapy services.


- A Group Programme for Dialectical Behavioural Therapy.


Training Opportunities:


-KUF Awareness Level Personality Disorder Training.


-Therapeutic Community and Therapeutic Environments Practitioner Training.


-5 Day Bespoke Programme specifically tailored towards this pathway which includes Ruptures and Repairs, Mentalisation Based Therapy, DBT, Risk Management, Crisis Planning and Contingencies clinical skills workshop.


-Dialectical Behavioural Therapy Full Training Programme
