The East Cornwall locality includes GP practices in Liskeard, Pensilva, Launceston, Saltash, St Germans, the Rame Peninsula, Looe, Callington and Gunnislake. East Cornwall is the largest of the 9 localities and has a registered population of approximately 105,000 patients.

There are 9 GP practices in the locality and patients generally use University Hospitals Plymouth NHS Trust for their acute hospital needs. The locality has a good track record of delivering service improvements and commissioning local services.

East Cornwall is a Primary Care Home site. The locality is committed to developing an integrated care team approach to better support people within the community setting. This will bring together a range of health and social care professionals to work together to provide enhanced personalised and preventative care for their local community.


We are looking for a highly motivated, enthusiastic and adaptable Clinical Pharmacist Locality Lead to work across our practices whilst hosted by Kernow Health CiC.

The post holder will work strategically across the east Cornwall locality whilst working within one of our locality general practices, who are currently providing primary care services to a registered practice population across a number of sites and who are part of the NHS England Wave 5 Clinical Pharmacist scheme.

This is an exciting portfolio role to provide clinical pharmacist medicines optimisation leadership to our locality whilst providing clinical pharmacist support to one of our practices within the locality.

The role will provide leadership and expertise in supporting the delivery of the medicines optimisation schemes across our GP practices. As the clinical pharmacist locality lead you will support the strategic planning and oversee delivery of core medicine optimisation projects whilst providing one practice with part time clinical pharmacist capacity. This element of the role will support the practice with prescribing processes that deliver high quality and safe medicines optimisation and cost effective prescribing to patients.
