Warrington Together is the borough’s main health and social care bodies working together to explore ways of delivering improved, integrated services to the people of Warrington.

We have an exciting opportunity for an Occupational Therapist or Physiotherapist to join our newly developed Band 6 rotations within the Acute Therapies Service. These roles are an exciting opportunity for you to be part of our new system wide model of care for our Frail population - as part of the boroughs Warrington Together programme.

The rotations include: the Rapid Response Team and our newly opened Frailty Assessment Unit.

The Rapid Response Team is primarily an assessment and discharge service – the post holder will be required to work holistically carrying out the role of both OT and Physio.

This is a unique opportunity to develop specialist skills using a therapy model that delivers responsive solutions to a needs led assessment enabling our patients to be placed on to the correct care pathway.

The Frailty Assessment Unit –the post holder will gain increased knowledge and experience of working within this specialist area as part of the Multi-Disciplinary Team. Our philosophy will be “Home First”, whenever possible, with an emphasis on the person’s preferences, personal strengths, self-care, consultation and inclusion in every stage of their care planning. When home first is not the best option, frail older will be identified, assessed and offered a pathway of care which aims to get them up, moving, independent and safe and comfortable enough to return to their usual place of residence as soon as possible.

Therapies are actively involved in a number of AHP strategy improvement projects including the “NHSI AHPs Supporting Patient Flow Collaborative” and the Trust has just launched their first trainee advanced practitioner posts. We are looking for talented individuals who are committed to working as part of our Therapy Team delivering an excellent, caring service to patients.

Our ideal candidate will be confident, have appropriate NHS training and will be clearly able to demonstrate a desire to actively participate in all aspects of the role. The successful candidate must be an effective team player with good communication and organisational skills. They will be able to demonstrate clear clinical reasoning and have the ability to explain their decisions and actions in a way that all concerned will be able to understand.

The working hours for this post will be 37.5 hours - over 5 days working days which, on some rotations, will include Saturday and Sunday working.

Current employees of Warrington and Halton Hospitals NHS Foundation Trust who have had a satisfactory DBS check, carried out by this Trust during the last 3 years, will not be required to undertake a new one. Where staff have not had a check (classification appropriate to the post offered) within the last 3 years, they will be required to undertake and pay for this, as per the arrangements above.
