OUR TRUST VALUES

Our Trust is a great place to work and we are keen to ensure that all our employees have the right values in order that we can provide the best service to our patients. These values are very important to us, and as a potential candidate you should be made aware that we test for these throughout the whole assessment/recruitment and selection process, and will only interview and recruit individuals who demonstrate these values through their application and behaviour.

Details of our Values can be found on our website link 'Working together for outstanding care - our vision'

In line with the Five Year Forward View and the Herefordshire and Worcestershire Sustainability and Transformation Partnership, South Worcestershire Alliance has a strong local focus and commitment to secure new, sustainable models of care via the establishment of integrated community teams, known as Local Neighbourhood Teams.

We believe that the introduction of Local Neighbourhood Teams provides the essential foundation upon which to build and develop out of hospital care and improve patient experience – with teams built around GP practices.

Our vision is to secure Local Neighbourhood Teams with ‘one team identity’ working to an agreed set of goals and priorities regardless of who employs different staff.  This will improve coordination of care and communication and reduce duplication.

This is an exciting opportunity for a dynamic Healthcare Assistant to work within a busy and developing team.

The post holder will carry out visits within the community to assist the Team in preventing hospital admission but also to support early discharge from hospital. The post holder will be expected to carry out tasks as detailed on Care Plans including support with personal care, medications, exercise and basic Nursing interventions.

You will be rostered on to cover the rota for the Team as required. This will be 5 shifts a week across Bank Holidays and weekends and working a mix of Early (0730-1500) and Late (1430-2200) shifts.

You will need to be flexible and work well within a team as well as work autonomously and deliver the required treatment for a wide range of patients with a wide range of conditions.

We aim to deliver an effective, high quality service that puts patients at the centre of their own care.  You will work closely with GPs, Social Care, the Voluntary and private sector, Community Therapists, Advanced Nurse Practitioners, working across the planned, rapid response and proactive pathways.

It is essential applicants can meet the travel requirements of the post.

For further information, please contact Jo James, Operational Lead on 01905 681715.

PLEASE NOTE: All new appointments are subject to 6 months’ probation period.

If the same or similar vacancy arises within the recruitment period of this post or up to 3 months afterwards, the Trust may chose not to re advertise, but to select a suitable candidate from this interview pool. The new post may differ in terms of the base, hours available (full or part time) or contract type (permanent or fixed term) and therefore you are encouraged to apply for advertised posts but specify on your application form your preferences.

In the event of exceptionally high levels of response, we reserve the right to close the post before the date stated in order to prevent the number of applications received being unmanageable. You are advised to submit your completed application as soon as possible to have the best chance of being considered.

Applicants must have the ability to communicate clearly and effectively in English in both verbal and written formats.

**If the position being applied for requires a DBS check to be undertaken on you, it will be a condition of your employment that you have subscribed and continue to subscribe to the DBS Update Service. New employees to the Trust are required to pay the fee for the initial DBS check (this will be deducted from their first months’ salary) and the annual subscription (£13) to the DBS Update Service. Internal applicants are required to pay the annual subscription fee only (£13). Further information on the update service can be found on our website link ‘DBS Update Service.’

The Trust offers a preceptorship year for newly qualified nurses and AHP's and attendance at preceptorship programmes is a condition of employment.
