Are you ready to become our lead therapist in Rapid Response in Hammersmith and Fulham? Are you a Physiotherapist or Occupational therapist ready for a new challenge as a Band 7. Then join us and be part of a committed and caring team on the next part of our journey to make Hammersmith & Fulham a great place to grow old. You will work with our dynamic rapid response team to deliver hospital discharges in the home first pathway and prevention of admission in our Rapid response pathway. You will be key in developing the service within CIS to deliver effective pathways of care through hospital to community, you will be key in relationship building with our hospital and community partners.


We are achieving excellent results in admission avoidance and reducing length of hospital stay for the people of Hammersmith and Fulham and are part of a wider community independence service offered in Westminster and the Royal Borough of Kensington & Chelsea. You will be working closely with our acute partners, such as Imperial College Healthcare NHS Trust and Chelsea & Westminster Hospital NHS Foundation Trust.


We will ensure you are able to follow your clinical and leadership development priorities identified through your personal development and appraisal plan.


You will be connected with peers across the triborough of Hammersmith & Fulham, Westminster and Kensington and Chelsea to support you and to work on service improvement partnerships.


We will equip you for mobile working and actively support flexible working practice.
