The team at Cherry Tree house are leading a ground breaking initiative to provide a high level of support for people with complex and long standing mental health issues that impact upon their ability to fulfil satisfying and meaningful roles.

 

We now require an innovative, creative and highly skilled Band 6 Community Mental Health Nurse to act as a care co-ordinator and to form a key part of the multi-disciplinary clinical team, whose hours are Monday to Friday 9am to 5pm.

 

The team at Cherry Tree House is linked to the Hounslow Treatment and Recovery team, and work closely with service users and carers to help maximise independent functioning, focusing on discharge to less supportive accommodation and actively promoting recovery. This role will also involve working to develop detailed CPA and health and wellbeing plans, and to provide evidence based interventions within a one to one or a small group setting. There will be opportunities to participate in and potentially lead on quality improvement initiatives and research.
