Registered Nurse Band 5 Jubilee House

Continuing Healthcare Assessment & End of Life care

We are looking for a dynamic, compassionate and caring Registered Nurse with a passion for delivering high quality care within a community in-patient environment. More specifically with an interest in providing high quality care for patients undergoing continuing health care assessment or those who maybe at the end of life.

The individual will be enthusiastic, motivated and committed with the ability to work hard as part of a team, in an integrated way with other disciplines across health and social care delivering high quality, holistic care. Sensitivity and empathy attributes are key.

Jubilee House is a 25 bed adult inpatient unit situated in its own grounds in Portsmouth. The unit provides care for patients undergoing assessment of long term continuing healthcare needs and provides end of life care to those patients admitted in the final stages of their illness.

Jubilee House promotes a multi-disciplinary team approach, working to ensure that patients are (where appropriate) discharged to the most suitable environment which will continue to support individuals with their long term needs.

At Jubilee House, care is provided 24 hours a day, 365 days a year. As part of our commitment to provide consistent care, the post holder will be expected to be part of an internal shift rotation pattern as required, in order to meet the needs of the service.
