An exciting opportunity is available for a Senior Mental Health Practitioner to work as a valued member of the Primary Care Networks. The role will include providing specialist mental health care working with primary and secondary care and the neighbourhood teams to locally support the mental health transformation and improve the lives of people who access healthcare services.

The successful candidate will be an experienced clinician with proven skills in working with people who have a range of highly complex mental health needs, demonstrating practice that is evidence-based and innovative. They will demonstrate adaptability, excellent communication skills and self-motivation.
In addition to undertaking mental health assessments, you will take on a caseload of highly complex service users to provide therapeutic interventions and coordinate their care.
Within the role you will be based within the locality Integrated Place Based Team and provide mental health leadership and implement clinical processes. The role will involve providing specialist clinical knowledge and interventions to service users in primary care setting to support service users with highly complex mental health needs/serious mental illness.
Alongside this the role will provide best practice training, mentorship and guidance to mental health practitioners and other roles within the primary care/ neighbourhood MDT.

Whilst a full Job and Person specification is attached to this advert, key elements of the role will include (but not be limited to):

• Providing specialist clinical knowledge and interventions to service users in primary care setting. This will include working with the practice team to support people with highly complex mental health needs/serious mental illness.
• Developing and implementing clinical processes that contributes towards the continuous improvement of mental health provision across the primary care network (multiple GP practices)
• Providing best practice training, mentorship and guidance to mental health practitioners and other roles within the primary care/ neighbourhood MDT
• Managing a caseload of highly complex/SMI presentations working closely with the locality secondary mental health service, to maintain personalised care plans which focus on what matters to the person and avoid escalation to.

The Successful Candidate will:
• Have the qualifications and experience as highlighted in the Person Specification.
• Be an experienced clinician with proven skills in working with people who have a range of highly complex mental health needs, demonstrating practice that is evidence-based and innovative.
• Preferably have some experience of working closely with or within primary care.
• Demonstrate adaptability, excellent communication skills and self-motivation.
• Be able to demonstrate appreciation of people’s needs and a track record of improving care
 The post holder will be encouraged to contribute to service development, and there will be opportunities for CPD and clinical supervision as professional development is actively encouraged.
