Diabetes Specialist Nurse for housebound patients

The prevalence of diabetes in Staffordshire is significantly increasing. Some of the increases are due to improvements in awareness, early diagnosis and recording however based on current obesity trends, the prevalence of diabetes is predicted to continue to increase. Furthermore people with diabetes are living longer and may require additional support to manage their diabetes within their own home.

 

Within The South East Alliance we have an exciting opportunity for a Diabetes Specialist nurse to join our Portfolio to support people with diabetes who are housebound, either within their own homes or within a residential setting. We are recruiting dedicated, compassionate staff to further enhance the high quality patient experience we offer within the diabetes service.

 

The successful applicant needs to be educated to degree level and have a post registration qualification in diabetes care. Experience of working with people with diabetes and their families will be required alongside an understanding of current diabetes policies. You will need to be able to work on your own initiative without direct supervision and also be able to work well within a team.

 

This role will involve working across the community setting and you will need to be able to travel across Burntwood, Lichfield and Tamworth.Therefore a driving license and access to a car is essential.

 

This is an exciting time to join the team and you will be involved in the design and implementation of the dedicated housebound service from the beginning.
