NHS Dorset’s Personal Health Commissioning service is growing and so has some new and exciting opportunities for nurses and health and social care professionals.

The Ongoing and Review team are seeking experienced registered nurses (Adult, Mental Health or Learning Disability) or Health and Social Care professionals motivated by providing outstanding person-centred support.

As a result of an expanding service and investment, we currently have two part time Senior Practitioner positions available working in the East locality. The service operates Monday to Friday between the hours of 9-5. NHS Dorset puts emphasis on work life balance and therefore flexible working requests will also be considered in line with the CCG policy. Currently the service is working remotely due to the Covid 19 pandemic however the need may arise to undertake community visits.

Working within a sound knowledge of the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care, the successful candidates will hold a caseload of individuals who are eligible for CHC funding and be able to demonstrate a strong person – centred approach. You will need to be able to undertake high quality care needs reviews, demonstrating an openness and honesty with the individual and their family or representative. You will work with commissioning colleagues to ensure packages of care are safe and sustainable and support an individual’s assessed needs. A knowledge of Personal Health Budgets would be advantageous. You will have a sound understanding of safeguarding principles and the Mental Capacity Act. As part of this role you will also undertake Community DOLS applications and be able to gain greater knowledge and skills in working with this important legislation as it transfers to Liberty Protection Safeguards.

You will have excellent communication skills, be able to work collaboratively with health and social care partners across the Dorset system and have a flexible approach, be able to prioritise and work autonomously when needed.

Due to the nature of the role, there is a requirement to travel, and a full driving licence is essential. This role will also be the subject of an Enhanced DBS check.
