Are you looking for an opportunity to get involved in service reviews, design and commissioning of community services?  NHS Dorset Primary and Community Care Team, are looking for a highly committed and resilient individual to undertake the role of Senior Primary and Community Care Lead on a 12 month fixed term/secondment to lead on key pieces of work.

The portfolio is commissioning and contracting of community services and focus will be to review services related to intermediate care and support the system redesign and commissioning of the new model.

This senior role will be part of the commissioning and contracting team and will support the overall commissioning and contract management of community services.

To be successful in this role, you will need to have experience in reviewing the effectiveness of services using data and stakeholder engagement. You will also need to demonstrate strong and effective communication skills and have a track record building robust relationships across professional groups.

We are looking for a person with experience leading and managing service reviews at a senior level in a healthcare setting ideally with experience of Community services and experience in commissioning and contracting of new models of care.

You must be a person who is confident in working autonomously and comfortable with the use of data / information analytics.

You will also be a problem solver, at ease with project management methods, have a strong team ethos and lead the work with Providers to ensure a collaborative approach.

We pride ourselves on our supportive team ethos, our ability to successfully deliver on objectives and for the positive relationships we have built with our Providers, system colleagues and the wider community system.

This position can be based at Dorchester or Poole with working from home as a hybrid working pattern with a frequent requirement to travel across Dorset. There is a requirement to occasionally attend evening events.
