Personal Health Commissioning (PHC) has undergone a full-service re-structure and is continuing on a journey of ongoing improvement to ensure we are at the forefront of providing high quality, efficient and sustainable services for our population.

We currently have exciting opportunities for motivated and experienced registered nurses to join our team as Senior Practitioners to support in undertaking detailed assessment and reviews of Continuing Healthcare eligibility for individuals applying for and already in receipt of CHC/Funded Nursing Care packages. We currently have 2 full time Senior Practitioner positions available. The service operates Monday to Friday between the hours of 9-5. Whilst these posts are full time, a job share may be considered and the CCG puts emphasis on work life balance and therefore flexible working requests will also be considered in line with the CCG policy.

You will work within a strong and supportive team, playing a key role in making recommendations on eligibility for CHC as well as supporting the Operational Managers in the implementation of the national frameworks and other national policies as they relate to CHC/FNC. The CHC team are a supportive team with a wealth of knowledge, skills and expertise enhancing a robust peer development programme. Strong leadership encourages personal growth, learning and development and there are opportunities for learning and further study.

The successful candidate must be able to work flexibly and autonomously and work collaboratively with Health and Social Care practitioners, patients, families and carers, ensuring high quality assessments and timely service is delivered.The practitioner must demonstrate an openness and honesty and have strong assessment and communication skills and a knowledge of the National Frameworks for Continuing Healthcare, the Mental Capacity Act and Mental Health Act and Care Act 2014.

We have 2 offices based in Poole and Dorchester and you may be required to provide support across these sites as necessary. There will also be a requirement to make off-site visits to undertake assessments and attend meetings and it is therefore essential that you have a full driving licence with access to a vehicle for business use.

Experience of line management is desirable but not essential.
