Care Co-ordinator

Dorking Healthcare (DHC) have had a busy and successful year and we are now recruiting for a motivated individual to join us as a Care Co-ordinator. The role will be based at Leith Hill Practice, Capel, Surrey within Dorking Primary Care Network (PCN).

The Care Co-ordinator will have a key administrative role in working with the practices and the wider primary/community care team to support patients with long-term medical conditions or complex health care needs. The Care Co-ordinator will work closely with clinicians to develop care plans and co-ordinate and monitor a range of patient services. This will include being the co-ordinator of reactive (urgent care for those in need) and proactive (planned care to support people to live as independently as possible) services for patients.

This is a developmental role and the successful candidate will be fully supported in attending the accredited care coordinator education programme and training in supporting the individual needs of the people on your case load.

Full time working (37.5 hours per week) is required for this role.

To be successful in this role, you will need a positive and compassionate nature and enjoy providing an exceptional customer service. This role requires sensitivity and an understanding that patients with any medical needs, complex or general, should be treated with respect. For the administrative side of this role, you will need excellent literacy and numeracy skills, proven administrative and IT skills (including using various data sets) and experience of managing a complex and varied workload. You will ideally have had experience of working with the general public in a similar role within healthcare or in the public sector, leisure or voluntary sectors. Excellent interpersonal skills, effective communication and the ability to problem solve efficiently is also essential to ensure the smooth running of the service. The successful candidate will need to relish the challenge of working under pressure, as part of a thriving and growing team both within a GP practice and as part of the wider DHC organisation.

About Dorking Healthcare

DHC is a GP Federation, representing 105,000 patients providing a wide range of quality healthcare services including elective care, Talking Therapies, community healthcare and primary care across Surrey, We are registered with the CQC (Care Quality Commission) and we are an accredited AQP (Any Qualified Provider).

DHC is working at the centre of the local health system to evolve and deliver integrated care and that requires strong partnerships with many organisations. The PCN’s are critical to the changing NHS and are the focal point of all locally developed planning and service delivery.
