Due to career progression we are looking for band 5 nurses who are enthusiastic and committed to providing evidence based nursing care.

These posts will be based in Braunton, within the Ilfracombe and Braunton Nursing Teams. The successful candidates will have excellent communication skills and a desire to work in the community. We deliver a variety of services in both the patient’s home and other appropriate community settings. We work closely with all members of the health and social care team to ensure that our patients are supported and enabled to be as independent as possible. If you have experience of, or an active interest in community nursing, preventing hospital admissions, managing people in the end stages of their life, or with chronic diseases; then we have a number of exciting community nursing opportunities within our day team service that operates 9am to 5pm seven days a week.

There will be a robust induction process and additional training needs will be supported.

We support all members of the team to work autonomously and strive to provide a working environment where this encouraged.

We have two vacancies available, one substantive for 22.5 hours and one fixed term for one year to cover maternity leave- with secondment opportunities for 22.5 hours.

Our teams of care professionals work with patients and their families to support peoples’ independence, health and wellbeing. We provide support to avoid hospital admissions, and if an admission is necessary, we try to make each patient’s stay in hospital as short and effective as possible before working with them on a safe discharge home.

Our patients (through the Friends and Family Test) regularly report an average of over 95% satisfaction with our services.
