In the Portsmouth Rehabilitation and Reablement Team, we strive to make a difference to the health and wellbeing of our communities.
We do this by delivering personalised support to enable people to stay well by supporting peoples journey out of hospital and supporting people who are at risk of a hospital admission by developing and implementing their own plans for health and wellbeing. We do this by providing a;
You will work as a member of a multidisciplinary team alongside service users to promote independence and daily living skills. The team consists of Occupational Therapists, Physiotherapists, Nurses, Social Workers and Rehab Assistants who work together in an integrated way to provide co-ordinated, person-centred support to promote independence and to improve quality of life.
This role will demonstrate excellent communication skills, using them to engage, persuade or negotiate with clients with complex, at times challenging needs to ensure they are provided with the most appropriate care and support.
