Portsmouth Rehabilitation and Reablement Team (PRRT) is a Health and Social Care team responsible for supporting admission avoidance and reablement for the adult Portsmouth community.

This is a fast-paced team, where no two days are the same. We are looking for individuals who will make a real difference and are passionate about keeping people safe, well and out of hospital.

PRRT delivers personalised care, support and interventions to maximise independence and reduce further deterioration. We do this by providing a;

  • Reablement response to help support individuals achieve their goals, supporting them to regain skills, confidence and independence enabling them to do things for themselves.
  • Responsive service that is built around place, with wrap around support to optimally manage their needs at home and avoid unnecessary admissions

You will work as a member of a multidisciplinary team alongside service users to promote independence and daily living skills. The team consists of Occupational Therapists, Physiotherapists, Nurses, Social Workers and Rehab Assistants who work together in an integrated way to provide co-ordinated, person-centred support to promote independence and to improve quality of life.

This role will demonstrate excellent communication skills, using them to engage, persuade or negotiate with clients with complex, at times challenging needs to ensure they are provided with the most appropriate care and support.

Please note you must be a car driver with a valid driving licence and have access to a transport with appropriate business insurance in order to travel throughout the trust, to meet the needs of the service
