An exciting opportunity has arisen for a 30 hour substantive contact for the post of urgent care nurse within the urgent care nursing team, based at St Georges road, Barnstaple but covering the whole of the Northern locality.

We are looking for a keen, enthusiastic nurse with a passion for providing support and high quality nursing care to people in their own homes in times of crisis, to avoid unnecessary admission to hospital and also to facilitate and early discharge home from hospital.

The successful candidate will have a good knowledge of community services and be required to deliver a very high standard of care and often under pressure/busy unpredictable workload.

You will join a dynamic team and work with community teams responding to urgent situations and enabling people to remain in their own homes.

Our Team work across Northern Devon between the hours of 16:00-08:00. We provide an evening service, hours 16:00 -23:00 or 16:00 – 01:00 and a night service, hours 22:00 – 08:00, which includes weekends and bank holidays.

Our teams of care professionals work with patients and their families to support peoples’ independence, health and wellbeing. We provide support to avoid hospital admissions, and if an admission is necessary, we try to make each patient’s stay in hospital as short and effective as possible before working with them on a safe discharge home.

Our patients (through the Friends and Family Test) regularly report an average of over 95% satisfaction with our services.
