We are looking for a confident and motivated individual with good communication skills and experience working within a rehabiltiation setting, to join our Neuro Early Supported Discharge Team in the East locality of Southampton and West Hampshire.

This is an exciting opportunity to work across the Early Supported Discharge Teams, Snowdon at Home and The Community Stroke Team, which provide rehabilitation and care to patients in their own home following neurological events.

The Neuro Early Supported Discharge Team provides six-week intensive neurorehabilitation to individuals with a Stroke or Neurological diagnosis, who have been newly discharged from hospital or are at risk of admission into hospital and have a GP within Southampton City or parts of West Hampshire.

The multidisciplinary team comprises of Physiotherapy, Occupational Therapy, Speech and Language Therapy, Nursing, Clinical Psychology and Rehabilitation Assistant therapy, which is delivered in a client’s own home or alternative community setting. In the role you would carry your own and a joint caseload of patients with supervision from registered Therapists including Speech and Language therapy, Occupational therapy, Physiotherapy and Psychology, whom you would work with to identify and support to achieve rehabilitation goals.

For this post, you will need to hold a full UK driving licence and have access to the use of a car. The post will be a job share across the East patch of the Teams Southampton and West Hampshire geography. Preferred standard working hours will cover the beginning of the week (eg. Mon to Wed).
