We are looking for a Team Lead to operationally lead a responsive and developing Urgent Community Response (UCR) team in Portsmouth. This role is a really exciting opportunity and the post holder will be instrumental in supporting the development of the Urgent Community Response Team, with a focus on reducing ED admission and unnecessary hospital conveyance.

UCR is provided by a multi-disciplinary team, including nurses, allied health professionals, social workers, and delivers urgent holistic assessment and intervention in the community to individuals who are at risk of hospital admission. The aim of the team is to avoid hospital admissions and enable people to live independently for longer. Through the UCR team, older people and adults with complex health needs who urgently need care, can get fast access to a range of health and social care professionals within two hours.

The successful applicant will work within a multi-disciplinary team. Applicants must be able to demonstrate an ability to manage and effectively lead a busy and dynamic team. This is an ideal opportunity for a practitioner to join a developing service, and be part of an evolving team reducing admissions to hospital and supporting more individuals at home. The service development is inline with the NHS Long Term Plan (2019) and the urgent community response standards.
