The Surrey Continuing Healthcare Team are looking for Band 7 ICP Locality Lead to join the Team.

The Surrey NHS Continuing Healthcare team covers Surrey Heartlands and Frimley Clinical Commissioning Groups part of Surrey Heartlands Integrated Care System.

Our team’s office is currently based in Weybridge with mobile working conditions.

As a ICP Locality Lead you will be responsible for providing high quality clinical presence, support, advice, training and development of staff competence in relation to

  • the Teams activities and reporting on performance
  • The CHC referral process, co-ordination of assessment of individuals, ongoing review, negotiating with providers and allocation of resources for the eligible individuals.
  • To develop systems and processes internally within the team working in partnership with multi agencies.

The post holder will use their knowledge and clinical skills to ensure that outcomes are determined in a consistent manner to ensure effective application and interpretation of the National Framework for NHS Continuing Health Care and NHS Funded Nursing Care (2018).
