We continue to develop our services to be increasingly integrated and place based. We have an exciting opportunity for Integrated Service Matron leadership roles within locality teams based in Southampton.

 

Solent NHS Trust and Southampton City Council have provided integrated services in Southampton, for several years and we are looking to strengthen and widen this relationship; having a positive impact on residents and patients in the city.

The Integrated Service Matron role is key to this development. Initially the matron will oversee clinical delivery of Community Nursing at a locality level, working with the other matrons jointly across all three localities in Southampton. As we deliver our “Futures” program, as part of the ongoing development of place-based care, Community Nursing will integrate with our Community Independence Service with clinical delivery models looking at the wider aspects of patient/resident support. As a key member of the leadership team you will be supported by your peers, Consultant Community Nurse, Operational managers, and Integrated Service Manager.

We are looking for registered nurses who have a background in community nursing with strengths around the change management agenda. Ideally the candidate will have experience in working in a multi-disciplinary team. Excellent interpersonal skills are essential in this role as are the organisational values around compassion and fairness. You will work closely with your peers within the sister locality services but also with the wider Southampton system including primary care/third sector to maintain a systematic and consistent approach to patient/resident care, focusing on safe practice and providing a quality integrated service. The role will be challenging and rewarding. You will have a direct influence on shaping the future of services within Southampton and wider.
