Motivated band 3 administrator required to join our friendly and supportive team.

Substantive - 25 hours a week Monday - Friday

The Pathfinder Team is a specialist community team based within North Devon District Hospital, consisting of professionals from therapy, nursing and paramedic backgrounds. Our remit is to holistically assess and then procure onward care, give advice, issue equipment and support with therapy needs for specific patient groups including:


  • Admission avoidance
  • Discharges from the short stay wards
  • Pathway 3 discharges (to nursing or residential homes)
  • End of life care discharges

We are looking for an administrator to join our team to support with the day-to-day smooth running of the office. Typical tasks would include answering the telephone, supporting patients and families with their enquiries and directing them to the most appropriate member of staff when required. You would also be needed to support with administration surrounding taking new referrals and adding these to the Trust electronic healthcare system, collecting data and making onward referrals to other teams. You would play a key role in arranging and writing minutes for team meetings and supporting with the administration associated with quality improvement projects, the recruitment process and new starter inductions.

We require a forward thinking, dynamic, motivated and creative individual to join us. Being able to work as part of a team and under pressure are essential skills that the successful candidate will hold. Experience with basic IT systems is essential, however training will be provided for the trust specific computer systems. We are a friendly, flexible and supportive team, who face challenges together and ensure we support each other through good times and difficult times.
